·   Completion of recognised screening tools, eg: Vanderbilts ADHD scales by parent/carers and teachers

·   School reports if available

·   Previous cognitive assessments if available

Correspondence from previous treating clinicians if available

· Strength and Difficulties questionnaire

Communication problems (Potential Autism spectrum disorders)

Ages 0 – 2 years

·   Copies of previous mental health assessments
