​Before your first appointment, we need to collect some information from you. Please check the link below on what to bring (or send via email or fax) prior to your child's appointment.

• Medicare card

• Referral letter from your GP if you have not already emailed or faxed it through

• Your child’s Health and Development Record (blue or green book)

• Any previous assessments or reports for your child
  (e.g. X-rays, blood tests, speech / psychology / cognitive assessments, school reports)

Please arrive with your child 10 mins prior to the time booked for your first appointment.

For acute or urgent referrals, please ask your GP to contact us directly. Your GP may need to discuss your case and arrange an early appointment.

CANCELLATION POLICY

Please contact the us at least 24 hours prior to your appointment if you are unable to attend and/or need to reschedule your appointment.
